I'm having malto/protein/creatine/leucine PWO, followed by about two cups of sticky white rice with a few tbsp. of butter. Kiefer suggests keeping the 1st meals fat free. You guys think the butter goes heavily against this recommendation or is it acceptable? The butter just makes it easier for me to eat that much rice. I remember him talking about hyper palatability in a podcast, where certain fats make it easier to eat more food, ie.....pizza because of the cheese.

You can do that, since you have carbs (malto) in the PWO Shake

On that note, does it mean that if we use a PWO shake with a decent dose of carbs, the shake counts as our first 'meal' and we don't have to worry about keeping any real food after that really low-fat? I'm assuming the fat is then supposed to gradually come in during the backload, with the fattiest meals closer to bedtime?

My take on this issue is that you should keep fat minimal until ~1 hour before your last insulinotropic meal.  For most fats(not MCTs), I've heard it takes ~2 hours to begin absorption, and clean insulin spikes last ~1 hour.  A few tablespoons of butter isn't going to do anything because even if you store the fat, it's such a small amount you wouldn't even notice.  Not to mention your body is probably going to be using fats for energy until you consume something in the morning.  So by keeping fat-loaded items within 1 hour of your last insulin-spiking meal, you're limiting the chances that the fat will be released before insulin sinks. There is also the fact that your body will probably be "focused" on storing glycogen rather than fat(I think Tex is alluding to this).  I'm not sure how true this is, but I have heard it before.A few tbsp of butter is fine with dinner, just don't start pounding the ice cream back until later.
