Just wanting to share that I've found chewing nicotine gum just before starting and during a workout has decreased my primary exertional headaches and seems to have no negative impact, even mildly positive impact on my lifting.Any thoughts on the mechanism of this?

The report says you can expect to have better lifts, so no surprise.  Could also be riding the  nicotine buzz and that's why you aren't getting the headache. The reason he suggested to take the nicotine a couple hours pre instead of during or right before is more for CBL.  You want the nicotine to leave your system before you take in carbs.

High glycemic carbs yes, good idea to keep separate. The whole chapter on why and what it causes.

Nicotine won't have fully left the system in 2 hours, but will not further inhibit tGLUT after that time (it will still be impaired however). So if we turn it back on via resistance training, insulin resistance won't be an issue.
